A man and his daughter and many other passengers were trapped on a high-speed train while the country was suffering from a zombie pandemic. The film revolves around survivors on a train from Seoul to Busan going through hardships, struggling to reach the last safe city after South Korea was infected by an unidentified virus.
Seok-woo, a fund manager who divorced his wife, is also a workaholic father who has a young daughter named Su-an. Her birthday the next day, she wanted her father to take her to Busan to see her mother. Father and son boarded the KTX 101 bullet train at Seoul Station. Other people on the same train include muscular husband Sang-hwa and pregnant wife Seong-kyeong, a high school baseball team, rich and arrogant Yong-suk director, two elderly sisters In-gil and Jong-gil, and a homeless man under stress.
When the train departed, a young girl, who was convulsing, boarded the train with a leg injury. She turns into a zombie and bites an attendant, then the attendant also turns into a zombie. The virus quickly spread throughout the train. Baseball player Yong-guk, a girl named Jin-hee - who falls in love with him, and some passengers have to escape to other cars. At that time, the TV reported on the zombie epidemic (but called violence) nationwide and the Cheonan-Asan Station was invaded by zombies so the train ignored it. After the train stopped at Daejeon Station, the surviving passengers found the place also filled with zombies, they rushed back to the train, splitting into groups in chaos. The train driver restarts the train to go straight to Busan, where the quarantine area has been established.
Seok-woo, Sang-hwa and Yong-guk fight to make their way to Su-an, In-gil, Seong-kyeong and the homeless man who is hiding. Together, they struggled through the zombies to reach the front carriage where the other passengers were hiding. However, due to Yong-suk's instigation, the passengers prevented the survivors from entering, for fear of becoming infected. Sang-hwa and In-gil sacrifice themselves to buy time for the remaining people to open the door and barge first. Yong-suk forces the newcomers to enter the next empty car, and they agree to follow his orders. After that, Mrs. Jong-gil deliberately opened the door for the zombies to rush in and kill all the passengers, including herself. Seok-woo, Su-an, Seong-kyeong, Yong-guk, Jin-hee and the homeless man are safe because they are in the next car. Yong-suk and the flight attendant escape from the zombies by hiding in the toilet.
The train was blocked at the East Daegu Station forcing the train to stop and the survivors had to look for another train. Yong-suk escapes after thrusting the hostess into zombies, then he does the same thing to Jin-hee. Heartbroken, Yong-guk stayed with Jin-hee and was bitten by her. The train driver starts a locomotive on another track, but while trying to save Yong-suk, he is also killed by a zombie. The homeless man sacrificed to give Su-an and Seong-kyeong time to run with Seok-woo to the locomotive that the train driver had started. They caught Yong-suk in the control room, he was bitten and was in the process of turning into a zombie. Seok-woo throws Yong-suk on the road, but he himself is bitten. He took Su-an and Seong-kyeong into the control room and confided his last words to his daughter before running out. When he was about to turn into a zombie, he remembered the first time he carried his daughter in his arms and fell off the locomotive with a smile
Su-an and Seong-kyeong get off in Busan and walk through a train tunnel. On the other side of the tunnel are soldiers who are stationed to protect the area from zombies. Unable to see the two newcomers clearly, the guards in the guard position received an order to shoot them. However, the soldiers then heard the singing, which helped them to know the two newcomers were human. Su-an tearfully sings the song she wants to sing to her father at the beginning of the film, and now sings it to honor his sacrifice.
